Trying to identify a Wild Plant?

I live in Gettysburg PA and I found a plant today while out in the woods, I can't identify it. It grows in patches and looks like it's a spreading plant that sends out runners. It is about a foot tall. Some plants have one stalk that look similar to that of a dandelion (light green/white and shiney). There is one leaf at the end of the stem and it has between 4 and five lobes.Other plants split into two branches and at the junction there is a five-pedal flower with yellow parts in the center.

This is probably a long shot by maybe someone can help. Thanks.
